Healthcare Fraud in Pakistan: The Unseen Danger Behind Vision Loss and Patient Harm

1. The Core Context: Fake Injections & The Avastin Crisis

When discussing healthcare fraud causing vision loss in Pakistan, the most prominent real-world case involves the off-label use of counterfeit or contaminated Avastin (Bevacizumab) injections.

  • How the Fraud Occurs: Avastin is an expensive, FDA-approved cancer drug that ophthalmologists frequently use off-label in micro-doses to treat diabetic retinopathy and age-related macular degeneration. Fraudulent distributors, unlicensed pharmacists, or negligent suppliers attempt to maximize profits by purchasing large vials, altering them, or compounding them into smaller syringes in unsterile, unapproved laboratory environments.
  • The Result: Contaminated, diluted, or counterfeit batches administered directly into the patient's eye (intravitreal injection) cause endophthalmitis—a catastrophic, rapid-onset bacterial infection inside the eyeball that frequently leads to total, irreversible blindness.

2. Authentic Categorization of Healthcare Fraud in Pakistan

Healthcare fraud in Pakistan operates across three distinct levels:

A. Quackery (Unlicensed Medical Practice)

  • Context: Thousands of individuals without formal medical degrees or specialized training open unauthorized street clinics, eye camps, or dental centers across urban and rural areas.
  • Mechanism: Performing minor eye procedures, prescribing dangerous steroids, or conducting unsterilized surgical extractions without a valid Pakistan Medical & Dental Council (PMDC) license.

B. Pharmaceutical Fraud & Spurious Drugs

  • Context: The sale and distribution of counterfeit, expired, or unapproved pharmaceuticals.
  • Mechanism: Distributors sell fake eye drops, expired antibiotics, or improperly cold-chained medications. The Drug Regulatory Authority of Pakistan (DRAP) regulates these substances under the Drugs Act, 1976, making the manufacture or sale of spurious or adulterated drugs a severe, punishable crime.

C. Institutional & Procurement Misconduct

  • Context: Financial corruption within public and private medical facilities.
  • Mechanism: Purchasing low-grade, unsterilized, or uncalibrated medical tools (e.g., phacoemulsification machines for cataract surgery) via corrupt procurement deals, leading to mass surgical failures or infections.

3. The Legal and Regulatory Framework

Victims and legal authorities in Pakistan rely on specific regulatory bodies and statutes to investigate and penalize health fraud:

Authority / StatuteLegal Basis & Mandate
Provincial Healthcare Commissions
(PHC, SHCC, KPHCC)
Established by provincial legislation (e.g., Punjab Healthcare Commission Act 2010). They inspect facilities, seal illegal/quack clinics, enforce standard operating procedures (SOPs), and issue administrative penalties.
Pakistan Medical & Dental Council (PMDC)Federal statutory body regulating doctor credentials. Handles disciplinary cases for professional misconduct under the PMDC Act.
Drug Regulatory Authority of Pakistan (DRAP)Governed by the DRAP Act, 2012. Regulates drug registration, quality control, manufacturing standards, and recall of contaminated products.
Pakistan Penal Code (PPC)Section 337: Causing hurt or physical injury by a rash or negligent act.
Section 304-A: Death caused by rash or negligent act.
Section 274/275: Adulteration and sale of drugs.

4. Red Flags & Preventive Measures

  • Check PMDC Registration: Every practicing physician in Pakistan must possess an active PMDC registration number.
  • Inspect Packaging: Intravitreal injections or eye drops must carry official DRAP registration numbers and tamper-evident seals.
  • Demand Written Records: Authentic facilities issue detailed, printed medical histories, diagnostic reports, and stamped receipts—never handwritten scraps or unverified cash-only receipts.
